Potomac Hospital participates with the following Anthem plans. Some restrictions apply.

Please note that effective August 4, 2003, Anthem reference lab patients (outpatients referred for lab work only) will no longer be able to have lab work done at Potomac Hospital. The only exception is for pre-operative testing. For Anthem Healthkeepers HMO, pre-operative lab testing may not be done at Potomac Hospital. Please refer to your Anthem Provider Directory or Anthem's web site for participating reference lab providers.

• Anthem BlueCross FEP PPO
• Anthem BlueCross of Virginia PPO: Authorization is required for MRI/MRA, CT, and nuclear cardiology procedures.
• Anthem Healthkeepers HMO: Authorization is required for HMO/Keycare POS members for MRI or MRA procedures. Pre-operative lab testing may not be done at Potomac Hospital.
• HMO/Keycare POS: Authorization is required for MRI or MRA procedures
