Potomac Hospital is not authorized to provide outpatient diagnostic services to Kaiser members in the following Kaiser plan:

• Signature HMO

Restrictions apply for outpatient laboratory, radiology, and cardiology services performed at Potomac Hospital.  Referral and/or authorization from the primary care provider are required for all plans except:

• Flexible Choice Option (PHCS) 3 – using out of network benefits*

*Authorization may be required for certain CT scans and nuclear medicine/cardiology procedures.
